**This amount is non-refundable after April 4, 2008. Request for refund must be in writing and received by April 4, 2008. An administrative fee of $200.00 will be deducted April 4, 2008 and before.

**If total is not paid by April 4, 2008, the person will be placed on space-available list subject to availability.
